Profhilo® Structura from IBSA Derma is the variant of the Profhilo range aimed at volume loss due to atrophy of facial adipose tissue. It concentrates 90 mg of non-cross-linked hyaluronic acid in 2 ml — 45 mg of high molecular weight and 45 mg of low molecular weight — stabilised by NAHYCO® thermal technology. Unlike Profhilo H+L, which acts in the dermal plane, Structura is applied with a cannula in the superficial fat compartment.

Composition and technology
- High molecular weight hyaluronic acid (H-HA): 45 mg per 2 ml syringe, 1,100–1,400 kDa.
- Low molecular weight hyaluronic acid (L-HA): 45 mg per 2 ml syringe, 80–100 kDa.
- Total concentration: 90 mg / 2 ml, equivalent to 45 mg/ml (4.5 %). This is the highest concentration in the Profhilo range.
- NAHYCO® technology: patented thermal process that combines the two molecular weight fractions by controlled heating and cooling. As the hydrogen bonds break and re-form, stable Hybrid Cooperative Complexes (Hybrid Cooperative Complexes, HCC) are generated. Without BDDE or any other chemical cross-linking agent.
- Rheological profile: G' of approximately 95 Pa. A deliberately low value compared with that of a volumetric filler: the objective is not mechanical projection, but integration into the fat compartment.
- Differentiating mechanism: the manufacturer describes an action restoring adipose tissue (lipolifting), aimed at the age-related atrophy of the facial fat compartments, and not at volume replacement by implant.

All three variants share NAHYCO® technology and the absence of a chemical cross-linker. They differ in concentration, application plane and pattern of points. Structura and H+L are not interchangeable: they treat different anatomical planes and can be used complementarily.
Indications and treatment areas
Indicated for the restoration of facial adipose tissue in patients with signs of structural ageing due to atrophy of the fat compartments.

Product for facial use only. The range does not currently provide a body indication for this reference; Profhilo Body is used for the body.
Technique and treatment protocol
- Injection plane: superficial fat compartment (subcutaneous plane), one layer below the dermal plane in which Profhilo H+L is applied.
- Technique: retrograde injection with a cannula, not with a needle. A pilot needle is used to create the entry point.
- "Deflation" pattern (predominant volume loss): 22G × 50 mm cannula, entry point approximately 2 cm from the tragus, 1 ml per hemiface deposited in the preauricular region.
- "Sagging" pattern (predominant laxity): 25G × 38 mm cannula, entry point approximately 3 cm from the tragus, 0.5 ml in the preauricular region and 0.5 ml in the zygomatic arch per hemiface.
- Dose: 2 ml per session, that is, one complete syringe (1 ml per hemiface).
- Protocol: two sessions with an interval of 30 days.
- Compatible with combined protocols within the IBSA Derma range; the sequence with cross-linked fillers or with Profhilo H+L should be planned according to clinical judgement.

Storage and pack contents
- Storage: between 0 °C and 25 °C, away from heat sources. Do not freeze.
- Do not use after the expiry date shown on the pack.
- Single-use product. Do not reuse or re-sterilise. Sterile and pyrogen-free contents.
- The pack contains: 1 pre-filled syringe of 2.0 ml, administration material, instructions for use and traceability labels for the patient record.
Contraindications and warnings
Manufacturer's warnings: do not inject intravenously, intramuscularly or into tendons. Do not mix with other products in the same syringe. It must not be used concomitantly with laser resurfacing treatments or with medium-to-deep peels. After injection it is recommended to avoid exposure to ultraviolet radiation for 3–5 days and to use SPF 50 sun protection.
Contraindications: known hypersensitivity to hyaluronic acid or to any of the components; active skin infection or inflammation in the treatment area; pregnancy and breastfeeding; patients under 18 years of age. Individual assessment in patients with autoimmune disease, coagulation disorders or anticoagulant therapy.
