Plinest® is the reference biorevitaliser from Mastelli S.r.l. (Sanremo, Italy), the company that developed and patented PN-HPT® technology (Polynucleotides High Purification Technology). It contains highly purified polynucleotides at 20 mg/ml — 40 mg per 2 ml syringe — the highest concentration in the Plinest range. It is not a filler: it does not provide volume, but acts on the fibroblast to improve the quality, density and elasticity of the skin.

Composition and technology
- PN-HPT® polynucleotides: 20 mg/ml, that is 40 mg per 2 ml syringe. This is the highest concentration in the Plinest line.
- Origin: trout gonads from aquaculture intended for human consumption. The material undergoes Mastelli's proprietary high-purification process, which removes proteins and contaminants of potential allergenic character.
- PN-HPT® (Polynucleotides High Purification Technology): technology patented by Mastelli, the company that introduced polynucleotides into aesthetic medicine. The polynucleotide chains retain a high molecular weight and a high degree of purity after sterilisation.
- Mechanism of action: the polynucleotides stimulate the dermal fibroblast to synthesise new matrix glycosaminoglycans and collagen and elastin fibres. As polyanionic polymers, they bind water and keep the extracellular matrix hydrated over prolonged periods, in addition to exerting an antioxidant action against free radicals.
- The formulation contains neither hyaluronic acid nor anaesthetic.

Plinest Fast, Plinest Eye and Plinest Hair declare the same PN-HPT® concentration (7.5 mg/ml) in the same 2 ml presentation: they differ by registered indication and by session protocol, not by the concentration of the active substance. Plenhyage XL Strong, which is marketed alongside this range, does not belong to the Mastelli family: it is a brand of Bioformula S.r.l.
Indications and treatment areas
- Loss of skin elasticity, firmness and density

- Scars, including the sequelae of acne
- Skin remodelling in areas with a higher fibrous content
Areas: face, neck, décolletage and dorsum of the hands.
Also used as skin preparation (priming) before laser treatment, chemical peeling, microneedling, radiofrequency or carboxytherapy, and in combination with other medical aesthetic procedures.
Technique and treatment protocol
- Injection plane: intradermal.
- Technique: microdeposits (multiple puncture) or linear retrograde threading with a needle.
- Needle: 30G ½" (13 mm); 2 units included. The consensus literature on PN-HPT® recommends 30 to 32G needles of 13 mm.
- Dose: 2 ml per session.
- Protocol: 3 to 4 sessions at intervals of 14 to 21 days.
- As a preparation protocol before another procedure, 1 to 4 sessions are used, starting approximately one month beforehand.

Storage and pack contents
- Storage: between 2 °C and 25 °C, protected from direct sunlight and heat sources. Do not freeze.
- Do not use after the expiry date shown on the pack.
- Sterile single-use product. Do not reuse or re-sterilise.
- The pack contains: 1 pre-filled syringe of 2 ml, 2 needles 30G ½", instructions for use and traceability labels.
Contraindications and warnings
Known hypersensitivity to any of the components; pregnancy and breastfeeding; active autoimmune diseases; active skin infection or inflammation in the treatment area, including active herpes simplex; coagulation disorders or anticoagulant therapy; ongoing oncological treatment; patients under 18 years of age.
As this is a product of fish origin, assess any history of fish allergy before application.
